Mark McMahon, Social Constructivism and the World Wide Web - A Paradigm for Learning, ASCILITE 97, http://www.ascilite.org.au/conferences/perth97/papers/Mcmahon/Mcmahon.html 7-10 December 1997

Am I going to work my way back to Vannevar Bush's Atlantic Monthly "As we may think" on the Memex? Perhaps. Working backwards, I first include Mcmahon's paper because it makes early mention of social constructivism in the context of education on the web. The paper is pre Web 2.0 and so doesn't really rave, as it might be today, about the web enabling a social constructivist approach to education.
